EFT for Medicare – CMS-588 Revised Form effective May 1: MAC’s will accept current version of the paper form through April 30, 2024. Starting May 1, 2024, you must use the revised paper form. If you are using PECOS to complete your EFT information, the new form will be embedded in the portal (no action needed).
